Lines and wrinkles

What it actually is

A dynamic line is a fold made by a muscle pulling. Stop the pull and the line stops appearing. A static line is one that has been folded often enough that the skin holds the crease at rest, and relaxing the muscle only softens it. Fine crepey lines across the cheek are neither: they are skin that has lost water holding capacity and structural support.

What causes it

  • Repeated muscle movement, which is why the first lines are where you are most expressive.
  • Collagen that reduces by roughly one per cent a year from the mid twenties, faster after menopause.
  • Ultraviolet exposure, which accounts for a large share of visible skin ageing and is the one factor you can still act on.
  • Smoking and sleep position, both of which put a mechanical fold in the same place night after night.

Before you book anything

Heavy upper lids that sit on the lashes are usually skin excess rather than muscle position, and that is an oculoplastic surgeon's job. We will tell you if that is what we are looking at.

Nothing on this page is a diagnosis. It is what we would say in the room, written down so you can read it before deciding whether to come in.

What treatment can do

Relax the muscles that are creating the fold, and separately improve the quality of the skin sitting over it so it creases less readily. Done together, the result is a face that still moves. Done as muscle relaxation alone, on skin that has thinned, the lines soften but the skin still looks tired.

What it cannot do

Nothing here removes a deep static line completely, and a clinic that says otherwise is selling you a filler that will look like a filler. Very fine lines under the eye are the hardest area on the face and the one where expectations need setting hardest.

What a course usually looks like

Muscle relaxing treatment lasts three to four months at first and often longer once the habit of movement has settled. Skin quality work runs as a course of two or three sessions a month apart, then a single session every six months. We include a two week review on every first injectable appointment, because adding a little more is easy and taking any away is not possible.

Course pricing is lower than the same sessions bought one at a time, and spreading the cost is available over three months on anything above £300.

Where to start

Bring a photograph of it at its worst.

Most concerns look different in clinic lighting at eleven in the morning than they do at home at night. A photograph on your phone is genuinely useful and we would rather see one than take your word for it.
